Case Manager

Job Title: Case Manager

Supervisor’s Title: Program Manager

General Summary: The Case Manager position is responsible for delivering comprehensive, client-centered casemanagement services to individuals and families residing at Hope Harbor. This role includes conducting assessments, developing individualized service plans, assisting clients with budgeting, providing education and referrals, and connecting clients to appropriate community resources. The Case Manager ensures that all services, interactions, and client progress are thoroughly documented in Clarity, the Homeless Management Information System (HMIS). The position supports clients in increasing stability, overcoming barriers to housing, and working toward long-term self-sufficiency while upholding Hope Harbor’s mission, values, and program standards.

Qualifications:

  • Must successfully pass all required background checks.

  • Associate’s Degree in Human Services or a related field required; Bachelor’s Degree preferred. Equivalent relevant experience may be considered in lieu of degree.

  • Current CPR and First Aid Certification, or the ability to obtain certification within thirty (30) days of hire.

  • Ability to follow written and verbal instructions accurately and consistently.

  • Strong communication skills, including clear and professional verbal, written, and typed communication.

  • Demonstrated reliability, punctuality, and strong time-management skills.

  • Ability to interact with individuals experiencing homelessness in a compassionate, trauma-informed, and dignified manner.

  • Ability to maintain strict confidentiality regarding client, staff, and volunteer information.

  • Able to work independently with minimal supervision while maintaining accountability and sound judgment.

  • Proficiency with computers, software applications, and data entry, including the ability to enter information accurately and meet required deadlines.

  • Ability to participate in an on-call rotation as assigned.

  • Ability to de-escalate crises and maintain professional boundaries.

Essential Duties and Responsibilities:

  • Provide Individualized Case Management

    The Case Manager provides intensive, client-centered case management by completing assessments, identifying needs, and developing individualized service plans with clear, achievable goals. They monitor and document client progress, including participation in Hope Harbor’s education program, and ensure residents follow program expectations. Responsibilities include assisting with employment readiness, supporting financial stability through budgeting and debt-repayment planning, and maintaining strong knowledge of local housing options and community resources. The Case Manager connects residents to services such as housing programs, medical and mental health providers, childcare, education, and financial assistance, and may facilitate life-skills or educational classes as assigned. All services are delivered using trauma-informed, strengths-based practices to support client stability and long-term self-sufficiency.

  • Link Clients to Community Resources

    The Case Manager links clients to a wide range of community services—such as housing resources, medical and mental health care, child care, employment and education supports, and financial assistance—and ensures clients receive guidance in navigating these systems. This includes staying informed about available local programs, coordinating referrals, and helping clients overcome barriers to accessing services.

  • Complete Accurate and Timely Documentation

    Document case management in digital and physical files. Track statistics on client entrance, exit, and program information. Follows expectations, timelines, and requirements of Homeless Management Information System data entry.

  • Cross-Training

    The candidate will engage in cross-training across departments to develop a comprehensive understanding of Hope Harbor’s programming through job shadowing, staff meeting participation, and effective communication.

  • Professional Skills 
    The candidate is expected to report to work on time or provide advance notice of absences, manage workload effectively, and complete high-quality work within deadlines with minimal supervision. Strong communication skills—both written and verbal—are required, along with active listening, sound judgment, and thoughtful decision-making. The candidate should take initiative, identify problems and develop solutions, and contribute positively to a respectful team environment, helping advance Hope Harbor’s mission.
